Job summary

Wilson Medical Center is seeking a compassionate Licensed Practical Nurse (LPN) for a 5x8+ evenings (3p-11p) rotating weekends/holidays, in-person care in a 90-bed Skilled Nursing & Rehabilitation Unit. The nurse will assess patients, initiate interventions, and document care plans with the interdisciplinary team.

Qualifications

State Licensed Practical Nurse with active license in North Carolina.
Graduate from an accredited nursing program and board-eligible/licensed.
Ability to perform patient assessments and document care plans accurately.

Responsibilities

Perform patient assessments and identify care needs.
Initiate appropriate nursing interventions and monitor responses.
Provide age-appropriate care and documentation of medical necessity.
Maintain and revise the Interdisciplinary Care Plan for each patient.
Complete timely QI assessments and ensure accurate records.
