Job summary

Wilson Medical Center in Wilson, NC is seeking an LPN to join a 90-bed Skilled Nursing & Rehabilitation Unit. The role includes evening shifts (3p-11p), rotating weekends and holidays, with a $15,000 commitment incentive.

As a member of the health care team, you will perform patient assessments, document care, and contribute to the interdisciplinary care plan for patients with CHF, COPD, diabetes and other conditions.

Qualifications

• Current state LPN license.
• Graduate of an accredited nursing program.
• BLS certification required within 30 days of hire.
• Long-term care experience preferred or desired.
• New grads welcome.

Responsibilities

• Perform patient assessments and identify patient needs.
• Initiate appropriate nursing interventions.
• Provide care appropriate to condition and age of the patient.
• Document medical necessity in the medical record.
• Complete and revise the Interdisciplinary Care Plan for each patient.
• Conduct timely QI assessments.
