Job summary

Ontario Center seeks a compassionate Licensed Practical Nurse to provide hands-on care in our skilled nursing and rehab facility. The LPN will deliver medications and treatments, monitor resident status, and support restorative therapies in a resident-centered environment.

Responsibilities include assisting with ADLs, documenting care, collaborating with an interdisciplinary team, and promoting safety, dignity, and comfort.

Qualifications

• Current California LPN license is required.
• Experience delivering medications and treatments preferred.
• Ability to document care accurately in charts or EMR.

Responsibilities
