Job summary

CareRite Centers seeks a compassionate Licensed Practical Nurse to provide resident-centered care in our skilled nursing and rehabilitation facility. The LPN delivers bedside nursing, administers medications and treatments, monitors resident conditions, and documents in EMRs.

You’ll collaborate with an interdisciplinary team to implement individualized care plans, support rehab and long-term care goals, and educate residents and families.

Qualifications

• Active LPN license with TN state approval.
• Minimum state-required nursing licensure for LPN practice.
• Experience in skilled nursing/rehabilitation settings preferred.

Responsibilities

• Provide bedside nursing care to residents.
• Administer medications and treatments.
• Monitor resident conditions and document in EMRs.
• Collaborate with interdisciplinary team to implement care plans.
• Educate residents and families about care plans and health maintenance.
