Job summary

CareRite Centers seeks a compassionate Licensed Practical Nurse to provide resident-centered care in our skilled nursing and rehabilitation facility. The LPN delivers bedside nursing, administers medications and treatments, and monitors resident conditions.

You’ll collaborate with an interdisciplinary team to implement individualized care plans, support rehab and long-term care goals, and educate residents and families.

Qualifications

Active LPN license in good standing.
Experience in skilled nursing or rehabilitation setting preferred.
Strong communication and documentation skills in EMRs.
Ability to collaborate with an interdisciplinary team.

Responsibilities

Provide resident-centered nursing care.
Administer medications and treatments.
Monitor residents' conditions and respond to changes.
