Job summary

Discovery Village at Alliance Assisted Living and Memory Care, part of Discovery Senior Living, seeks an experienced Licensed Practical Nurse (LPN) to provide compassionate nursing care aligned with each resident’s care plan. You will monitor health status, coordinate with the care team, and support safety and well‑being in a community setting.

The role requires an active LPN license, 2+ years of nursing experience, and comfort with electronic documentation.

Qualifications

• Current Licensed Practical Nurse (LPN) license or a Professional Registered Nurse as per state regulations.
• Minimum of two (2) years of Licensed Nurse experience.
• Experience in assisted living, long‑term care, or a skilled nursing environment preferred.
• Strong clinical judgment, problem‑solving, and decision‑making skills.

Responsibilities

• Provide nursing care to residents in accordance with established care plans, policies, and procedures.
• Conduct ongoing assessments of residents’ physical and psychosocial status and monitor for changes in condition.
• Administer medications and treatments as prescribed and document appropriately.
• Coordinate and communicate resident care needs with physicians, families, pharmacies, and other care providers.
• Support health promotion, disease prevention, and wellness initiatives within the community.
• Ensure accurate, timely, and compliant clinical documentation.
• Assist with care planning, service plan updates, and resident evaluations as required.
• Collaborate with caregivers, wellness team members, and leadership to ensure consistent, high‑quality care delivery.
• Respond to resident concerns, incidents, and emergencies using sound clinical judgment.
• Support training and guidance for care staff as appropriate.
• Ensure the community adheres to applicable federal, state, and local regulations related to resident care and services.
• Participate in quality assurance initiatives, audits, and compliance activities as needed.
