Qualifications

• Graduate from an accredited practical nursing program and become licensed as an LPN.
• An associate degree in practical nursing is preferred.
• Licensed Practical Nurse (LPN) license must be current and unencumbered.
• If IV therapy is required, IV therapy training or eligibility is needed.
• Completes required annual CEU and competency training.

Responsibilities

• Provides professional nursing care for residents in long-term care under supervision.
• Administers medications or treatments as ordered and monitors responses.
• Documents resident interactions and outcomes related to care, meds, behavior, and diet.
• Works with interdisciplinary care team to ensure health, safety and comfort of residents.
• Counsels residents and families on prevention and treatment plans.
• Educates residents to develop self-care skills.
