Prisma Health seeks an LPN for the Richland Hospital Nephrology Unit to provide patient-centered nursing care under the direction of a Registered Nurse or Physician. The role includes data collection, ongoing monitoring, and education for patients and families.

The unit specializes in peritoneal dialysis and telemetry, delivering acute care to medical and surgical patients. Responsibilities include documentation, med administration, and adherence to confidentiality policies.

Qualifications

• Education - Completion of an LPN program recognized by the State Board of Nursing.
• Experience - One (1) year experience as an LPN.
• In lieu of one (1) year of LPN experience, Prisma Health Nurse Residency Program or comparable residency may be accepted.
• In lieu of completion of an LPN program, program equivalency recognized by the State Board of Nursing may be accepted.
• In lieu of one year LPN experience, enrollment in an accredited LPN to RN transition program may be considered.

Responsibilities

• Provides nursing care for patients as delegated by the Registered Nurse/Physician.
• Monitors and documents patient status, including vital signs and lab values.
• Informs RN/Physician of activities performed and changes in patient status.
• Maintains accuracy and confidentiality of patient records.
• Collects data using specialized equipment and orients patients to routines.
• Administers and documents administration of medications.
• Performs other duties as assigned.
