Job summary

Elara Caring is seeking a Licensed Vocational/Nurse (LVN/LPN) to join our care team in Texas. You will follow individualized care plans, monitor patient progress, and work with the RN Case Manager to update plans as patient needs change.

You will document medications, communicate with physicians and families, and help prevent hospitalizations through proactive care. A valid license and a year of clinical experience are required; hospice/home health experience is a plus.

Qualifications

• State license as LVN/LPN required.
• Graduate of an accredited nursing program (Certificate, Diploma, or Associate).
• At least 1 year of clinical care experience.
• Ability to lift up to 50 lb and assist with patient transfers.
• Must have a valid driver’s license or reliable transportation for travel between homes.
• Hospice or home health experience preferred.

Responsibilities

• Follow each patient's care plan and physician orders.
• Monitor patient progress and report changes to the care team.
• Educate patients and caregivers on self-care techniques.
• Update care plans with the RN Case Manager as needs change.
• Document medication regimens and update profiles per guidelines.
• Communicate with physicians, patients, families, and the care team to ensure coordinated care.
• Help prevent avoidable hospitalizations through proactive patient care.
• Deliver compassionate, high-quality care to support positive outcomes.
