Qualifications

• Current State LPN License.
• High School Diploma or GED with 10 hours of continuing nursing education annually.
• New graduate LPNs or those with less than one year of experience referred to the LPN residency program.

Responsibilities

• Assesses patients and provides feedback to the Registered Nurse from data gathered to inform treatments and nursing interventions.
• Participates in developing nursing diagnoses and carries out the established plan of care and documents reassessments and patient responses.
• Carries out therapeutic interventions with safety, skill, and efficiency and documents patient responses.
• Evaluates patient responses to nursing care and therapies and revises the plan as needed in collaboration with the care team.
• Supports quality of care by adhering to parameters, participating in quality improvement, and maintaining a therapeutic, culturally competent patient relationship.
