Job summary

Okeena Health and Rehabilitation Center – Dyersburg, TN is hiring Licensed Practical Nurses (LPNs) for per diem or PRN roles in a skilled nursing and rehabilitation facility. You will provide direct resident care, administer medications, and support the clinical team to deliver high-quality care.

The position emphasizes strong clinical skills, teamwork, and the ability to work weekends and holidays as needed.

Qualifications

• Active LPN license in Tennessee is required.
• Experience in long-term care, skilled nursing, or rehab preferred.
• Strong clinical and assessment skills.
• Excellent communication and teamwork abilities.

Responsibilities

• Provide direct nursing care according to individualized care plans.
• Administer medications and treatments as ordered.
• Monitor resident condition including vital signs and changes in status.
• Complete accurate and timely documentation.
• Assist with admissions, transfers, and discharges.
• Work closely with RNs, CNAs, physicians, and therapy staff.
• Support residents with daily care needs when needed.
• Participate in care planning and family meetings.
• Follow infection control and safety procedures.
• Help train and support new nursing staff.
