Job summary

Better And Beyond Family Care in Cleveland, OH is seeking a compassionate Licensed Practical Nurse to join our Adult Day Care team. The role focuses on daily health monitoring, medication administration, and collaboration with the care team to implement individualized plans.

Ideal candidates have a valid OH LPN license, experience in adult/geriatric care, strong documentation skills, and CPR/First Aid certification.

Qualifications

• Valid Ohio LPN license in good standing.
• Experience in adult/geriatric care or day program is preferred.
• Knowledge of medication administration and clinical documentation.
• CPR and First Aid certification (or willingness to obtain).
• Excellent communication and interpersonal skills.
• Compassionate, patient, and dependable with a team-oriented attitude.
• Ability to multitask and remain calm in fast-paced or urgent situations.

Responsibilities

• Monitor and assess the health status of adult day care participants daily.
• Administer medications and treatments as prescribed by physicians.
• Maintain accurate patient health records and documentation.
• Coordinate with care team to develop and implement individualized care plans.
• Respond promptly to medical concerns, emergencies, or changes in participant condition.
• Provide health education and guidance to participants and their families.
• Ensure a safe, clean, and supportive environment for all participants.
