AA Healthcare Management's Adult Day Center seeks a dedicated Licensed Practical Nurse (LPN) to support participants' health, safety, and well-being in a lively, community-based setting.

The LPN will administer medications, assist with care plans, lead daily activities, accompany outings, and collaborate with families and staff to foster independence and quality of life.

This role emphasizes teamwork, adaptability, and compassionate care in a vibrant day program.

Qualifications

• Current Wisconsin LPN license
• Behavioral health and/or dementia experience preferred
• CPR and First Aid certification (or willingness to obtain)
• Strong interpersonal skills with participants, families, and staff in a positive manner
• Comfortable assisting with recreational and social activities including community outings
• Able to work independently and in a team
• Dependable, organized, and adaptable to changing needs throughout the day

Responsibilities

• Assist and formulate care plans for participants.
• Administer prescribed medications and injections as directed by physicians.
• Monitor participant health status, report concerns, and communicate with families and healthcare providers as needed.
• Document all participant care, interventions, and health-related updates accurately.
• Respond to emergencies and provide first aid when necessary.
• Assist in planning and leading daily activities to promote socialization, wellness, and cognitive engagement.
• Participate in and supervise community outings, ensuring participant safety.
• Support participants in maintaining independence, dignity, and quality of life.
• Collaborate with program staff, social workers, and families to meet holistic needs; provide guidance to staff.
