Job summary

CareCore at Meadows in Cincinnati, OH is seeking licensed practical nurses (LPNs) to join our skilled nursing and rehabilitation team. We offer full-time and part-time roles on all shifts, with a supportive, team-oriented culture.

Responsibilities include direct nursing care, administering medications, monitoring residents, accurate charting, and supporting STNAs and staff. Strong communication and compassion are essential as we follow care plans and state regulations.

Qualifications

• Active Ohio LPN license required.
• Long-term care experience preferred but not required.
• Strong clinical, communication, and teamwork skills.
• Compassionate and dependable work ethic.

Responsibilities

• Provide direct nursing care to residents in accordance with care plans and physician orders.
• Administer medications and treatments as prescribed.
• Monitor resident conditions and communicate changes to physicians and nursing leadership.
• Maintain accurate documentation and charting.
• Supervise and support STNAs and other nursing staff as needed.
• Follow facility policies, infection control standards, and state regulations.
