Qualifications

• Current LPN license in the state of employment.
• Graduate of an accredited practical nursing program.
• Strong communication and clinical skills.
• Ability to work both independently and collaboratively.
• Detail-oriented with effective time-management skills.

Responsibilities

• Administer medications and treatments as ordered.
• Monitor, record, and report changes in resident condition.
• Document assessments and care provided in the electronic medical record.
• Assist residents with daily needs as appropriate.
• Communicate effectively with residents, families, and interdisciplinary team members.
• Maintain infection control, safety, and confidentiality standards.
• Support the nursing team to ensure consistent, high-quality care delivery.
