Lutheran Senior Services seeks an experienced Licensed Practical Nurse (LPN) to deliver nursing care and coordinate resident services in a senior care setting.

The LPN will work with an interdisciplinary team to implement care plans, administer medications, monitor IV therapy, and document clinical information in the EMR, ensuring safety, quality, and regulatory compliance for residents.

Qualifications

• LPN licensure in state of practice.
• Must be CPR certified.
• Specialized training or experience in gerontological nursing preferred.

Responsibilities

• Practices nursing care within licensure and follows Nurse Practice Act; maintains current license and compliance with regulations.
• Collaborates with interdisciplinary team to create individualized resident care plans and ensures regulatory compliance.
• Administers medications and treatments; oversees IV therapy and nutrition as ordered.
• May initiate IV therapy if IV-certified.
• Documents care plans, assessments and medical information in the EMR per policy.
• Ensures a safe environment by addressing potential hazards and maintaining safety standards.
• Coordinates with resident services to ensure continuity of care and detailed reporting.
