Job summary

StoneBridge Senior Living is seeking a Licensed Practical Nurse (LPN) to join our elder care team. You will provide basic medical attention, administer medications, and assess residents’ wellbeing as part of a larger medical team in a 24-hour care setting.

Qualifications

• Must have a current state license to practice as an LPN, CPR certification, and potential additional nursing specialty certification.
• Knowledge of basic nursing practice with an interest in gerontological care.
• Prefer 2 years' experience in a long-term care setting.
• Basic computer skills and EMR experience preferred.

Responsibilities

• Coordinates nursing care for residents on the unit, ensuring assignments to a nurse aide capable of providing care.
• Provides safe and accurate medication-related interventions and documents them per schedule.
• Administers medications and treatments according to physician orders and current standards.
• Performs nursing assessments and notifies physicians of status changes.
• Prepares work assignment sheets considering resident safety and staff abilities.
• Reports staff shortages to nursing management and participates in performance discussions.
