Job summary

Pathways Healthcare is a home health and hospice provider committed to patient-centered, compassionate care in patients’ homes across Massachusetts.

We are seeking a compassionate Licensed Practical Nurse (LPN) to join our per diem team in Westwood with coverage across the Westwood, Quincy, Brockton, and surrounding communities. You will collaborate with RNs, physicians, patients, and families to ensure high-quality outcomes.

Qualifications

• Current and active LPN license in Massachusetts.
• Graduate of an accredited practical nursing program.
• Previous nursing experience preferred, especially in home health, hospice, or geriatrics.
• Proficient with electronic medical records (EMR).
• Excellent communication and interpersonal skills.
• Current BLS CPR certification.
• Valid driver’s license and reliable insured vehicle required.

Responsibilities

• Provide skilled nursing care under the supervision of a Registered Nurse and in accordance with the plan of care.
• Monitor and document patient condition, vital signs, symptoms, and responses to treatment.
• Administer medications and treatments as prescribed.
• Assist with implementation and ongoing evaluation of patient care plans.
• Educate patients and caregivers on medications, treatments, and disease management.
• Communicate changes in patient condition to the supervising RN and physician as appropriate.
• Maintain accurate and timely clinical documentation.
• Promote a safe and comfortable environment for patients.
• Coordinate necessary supplies, equipment, and services for patient care.
• Support quality improvement initiatives and compliance standards.
• Accept and fulfill assigned patient visits and schedules reliably.
