Job summary

Altru Health System is seeking a Licensed Practical Nurse (LPN) to perform and document assessments, manage health history and admissions, and participate in discharge planning. The role emphasizes patient-centered care and collaboration with a multidisciplinary team.

Responsibilities include supporting nurses and practitioners, ensuring timely documentation, and providing education to patients and families.

Qualifications

• LPN license with ND Board of Nursing or Compact State; eligible to start
• Minnesota LPN license within 12 months of start date
• Valid driver’s license with acceptable driving record
• Basic Life Support (BLS) certification within 90 days of start date

Responsibilities

• Assist licensed practitioner in identifying patient needs and care priorities.
• Carry out department standards of care and contribute to the plan of care.
• Collaborate with the multidisciplinary team to implement an integrated care plan.
• Delegate nursing interventions to competent personnel as appropriate.
• Identify early warning signs and implement prescribed nursing interventions.
• Communicate patient information with patients, families, and team members.
• Document patient progress and discharge education as required.
• Support discharge planning and patient education to optimize understanding.
