Job summary

Benchmark Senior Living in Connecticut is seeking a compassionate Licensed Practical Nurse (LPN) to deliver nursing care in a warm, home-like environment for residents.

You will supervise Resident Care Assistants, coordinate with care providers, and participate in wellness programs and benefits. An LPN license and CPR certification are required; 1–2 years of nursing experience with the elderly is preferred.

Qualifications

• Current state license as an LPN and CPR certification.
• Graduate of an approved LPN program (per state requirements).
• Minimum of 1 to 2 years of experience working as a nurse preferred.
• Previous experience working with the elderly in a paid or volunteer position is highly desirable.

Responsibilities

• Accurately assesses, communicates, and documents residents’ status.
• Observes and reports any significant changes in resident health to the care team per state regulations.
• Directs and supervises the daily work assignments of the Resident Care Assistants.
• Mentors, guides, and directs the Lead Resident Care Assistants in all aspects of their job.
• Coordinates care needs with community providers via an effective case management process.
• Balances service demands with supporting resident independence.
• Recognizes and assists in the prevention of elder abuse, neglect, and exploitation and reports per state regulations.
• Responds to emergencies and uses the personal emergency response system.
