You will collaborate with RNs and physicians, follow care plans, ensure safety, uphold infection control, and advocate for residents' well-being while delivering compassionate, patient-centered care.

Qualifications

• LPN license and active practice in Ohio.
• Experience in long-term care or assisted living preferred.

Responsibilities

• Provide basic nursing care and administer medications as prescribed.
• Monitor vital signs and report abnormalities to supervising RN or physician.
• assist with feeding, hygiene, mobility, and daily living activities.
• Maintain accurate EMR documentation and patient records.
