Qualifications

• Graduate of an accredited Practical Nursing program.
• Current LPN license in the state of practice.
• BLS certification preferred or required per facility policy.
• Recent LTC, SNF, rehabilitation, clinic, or acute care experience preferred.

Responsibilities

• Provide direct nursing care and implement individualized care plans.
• Administer medications and treatments as prescribed.
• Monitor, assess, and document patient conditions, reporting changes to the Registered Nurse or provider.
• Assist with admissions, transfers, discharges, and ongoing patient care.
• Maintain accurate and timely clinical documentation.
• Collaborate with physicians, Registered Nurses, and the interdisciplinary healthcare team.
• Follow infection prevention, patient safety, HIPAA, and regulatory standards.
• Educate patients and families within the scope of practice.
• Perform other duties as assigned.
