Job summary

Talented Medical Solutions is seeking a Licensed Practical Nurse (LPN) to provide compassionate, patient-centered care under supervision of an RN or physician. Administer medications, monitor conditions, and document care per individualized plans.

The role requires graduate of an accredited PN program with a current LPN license in the state of practice; BLS is preferred or required per facility policy. Experience in LTC/SNF, rehabilitation, clinic, or acute care is a plus.

Qualifications

• Graduate of an accredited Practical Nursing program.
• Current LPN license in the state of practice.
• Current BLS certification preferred or required per facility policy.
• Recent experience in long-term care (LTC), skilled nursing (SNF), rehabilitation, clinic, or acute care preferred.

Responsibilities

• Provide direct nursing care according to individualized care plans.
• Administer medications, treatments, and therapeutic interventions as prescribed.
• Monitor, assess, and report changes in patient condition to the Registered Nurse or provider.
• Assist with admissions, transfers, discharges, and daily patient care activities.
• Maintain accurate, timely, and complete clinical documentation.
• Collaborate with physicians, Registered Nurses, and the interdisciplinary healthcare team.
• Educate patients and families within the scope of practice.
• Follow infection prevention, patient safety, HIPAA, and regulatory standards.
• Perform other duties as assigned.
