Lunar Healthcare in Ohio seeks an LPN/RN to deliver nursing care in the Assisted Living Unit. You will monitor care delivery, administer meds, and ensure adherence to standards. You will receive and relay shift reports and coordinate with physicians and staff to maintain high-quality resident care.

The role requires current Ohio licensure (LPN or RN) and graduation from an approved nursing program. Experience with patient assessments, vital signs, and documentation is preferred.

Qualifications

• Current Ohio licensure as LPN or RN.
• Graduate of an approved LPN or ADN program.

Responsabilités

• Delivers nursing care in accordance with policies and procedures.
• Receives shift reports and documents resident care in notes.
• Administers medications and treatments as prescribed; monitor response.
• Maintains medication cart/room; check expirations and stores properly.
• Performs head-to-toe assessments with vital signs each shift.
• Documents events, changes in condition, and care provided.
• Coordinates with physicians, aides, and other staff to meet resident needs.
• Supervises Aides and ensures care aligns with Plan of Care.
• Assists with admissions, transfers, and discharges when needed.
• Participates in infection control and safety procedures.
