Mayo Clinic in Phoenix, AZ is seeking a Licensed Practical Nurse to provide nursing care under supervision, following the patient care plan and collaborating with the health team. You will participate in care planning and documentation in the EMR, mentoring others, and ensuring quality patient care.

Requirements include an LPN license and current BLS; daytime hours, Mon-Fri 7:30–16:30, with 5x8 hour shifts.

Qualifications

• Graduate of a school of practical nursing or passed LPN Boards.
• Current LPN license by applicable state requirements.
• Arizona: Maintains current Basic Life Support (BLS) for Health Care Providers.
• Additional specialty certification/training as required by the work area.

Responsibilities

• Provide nursing care to assigned patients under supervision.
• Collect data through observation and communicate information to assist RN/provider.
• Participate on the healthcare team and follow patient care plans.
