Job summary

Signature HealthCARE of Hart County Rehab & Wellness Center in Kentucky is hiring full-time LPNs for 12-hour day shifts with weekend rotation. You will provide direct nursing care, administer medications, and support residents with daily living activities to ensure comfort, dignity, and safety.

Qualifications

• Current, active LPN/LVN license in the state of practice.
• CPR/BLS certification preferred.
• Minimum one year of clinical nursing experience; long-term care experience preferred.
• Experience in a supervisory or charge nurse role is preferred.
• Familiarity with electronic health records (EHR) systems and basic Microsoft Office.

Responsibilities

• Provide direct nursing care to residents, including medication administration and wound care.
• Assist residents with activities of daily living (ADLs) while maintaining dignity and safety.
• Supervise and direct nursing assistants and medication aides during the shift.
• Monitor residents’ health status and report changes to physicians and families.
• Develop and review individualized care plans with the interdisciplinary team.
• Complete accurate clinical documentation per policy and procedure.
• Coordinate admissions, transfers, and discharges according to procedures.
• Participate in quality assurance, staff education, and regulatory surveys.
