Job summary

Signature HealthCARE of Sunrise is hiring a full-time LPN for day shifts to provide comprehensive nursing care to residents in a 137-bed facility in Kentucky. Competitive sign-on bonus and 12-hour shifts (7a–7p) with weekend rotation are offered.

Responsibilities include medication administration, ADL assistance, care plan development, and accurate documentation. CPR/BLS preferred; LTC experience and supervisory experience are valued. Benefits begin within 30 days.

Qualifications

• Current, active LPN/LVN license in the state of practice.
• CPR/BLS certification preferred (American Heart Association or American Red Cross).
• Minimum one (1) year of clinical nursing experience; long-term care experience preferred.
• Experience in a supervisory/charge nurse role preferred.
• Strong clinical observation, medication administration, and documentation skills.
• Knowledge of Federal and State long-term care regulations.
• Effective verbal and written communication skills.
• Ability to work independently, prioritize tasks, and exercise sound clinical judgment.
• Familiarity with electronic health records (EHR) systems.
• Proficiency with Microsoft Office applications.

Responsibilities

• Provide direct nursing care to residents, including medication administration and wound care.
• Assist residents with activities of daily living (ADLs), ensuring comfort, dignity, and safety.
• Supervise nursing assistants and medication aides per policy.
• Monitor residents' status and notify physician of changes.
• Develop, implement, and review individualized resident care plans with the care team.
• Ensure accurate clinical documentation per policy.
• Coordinate admissions, transfers, and discharges.
• Participate in QA activities, staff education, and regulatory surveys.
• Perform other duties as assigned by the Director of Nursing.
