Job summary

CarePartners Health Services in North Carolina seeks an LPN/LVN to provide skilled nursing care in the Day Center setting. You will triage participants, administer medications, assist with labs and procedures, and coordinate care with the provider and supervisor.

You will document patient status, deliver education on medications, and support the organization’s care goals. This role requires clinical competency and teamwork in a clinic environment.

Qualifications

• BLS certification must be obtained within 15 days of employment start date.
• First Aid certification must be obtained within 30 days of employment start date.
• Licensed Practical or Vocational Nurse (LPN/LVN) is required.
• Associate degree or vocational school graduate is required.

Responsibilities

• Triages participants and provides nursing care to promote comfort and wellbeing.
• Administers medications per MAR and facility standing orders.
• Assists provider and Day Center Supervisor with ordering labs and procedures.
• Performs wound care and changes dressings as needed.
• Documents care delivery, patient condition and progress; communicates with team.
• Provides patient education, especially regarding pharmaceuticals and adherence.
