Qualifications

• 0-2 years experience in a clinical or outpatient setting required.
• Licensed practical nurse licensure (LPN) in the state required.
• Basic Life Support (BCLS) certification required.

Responsibilities

• Perform patient intake, documenting symptoms, medical history, medications and vital signs in the EMR.
• Provide direct patient care, including administering medications and assisting with medical/surgical procedures.
• Conduct diagnostic testing (EKGs, basic labs) with proper sample collection and documentation.
• Ensure timely follow-up with patients via portal messages, calls and test results, following policies.
• Educate patients on treatment plans, medication management and preventive care.
• Collaborate with providers to support patient care plans and maintain HIPAA compliance.
• Maintain a clean, sanitized clinic environment and proper disposal of hazardous waste.
• Monitor and update daily logs (narcotics, refrigerator temps, vaccines, expiration dates).
• Serve as a liaison between patients, providers, and families with empathy and confidentiality.
• Perform other duties as assigned and maintain regular attendance.
• Comply with all policies and standards.
