Job summary

Sunrise Home Health is seeking a compassionate Certified Nursing Assistant (CNA) to support patients in the West Valley and surrounding areas. You will perform one-on-one care, assist with daily living activities, mobility, and personal care while collaborating with the nursing team.

You should have a high school diploma or equivalent, state CNA certification, and at least one year of direct patient care experience. Travel is required as needed.

Qualifications

• High School graduate or equivalent preferred.
• Proof of state CNA certification, if applicable.
• One year of direct patient care experience in an institutional setting.

Responsibilities

• Provide personal care and activities of daily living per written plan.
• Maintain a safe and healthy home environment.
• Plan and prepare nutritious meals; assist with feeding as ordered.
• Assist in ambulation and mobility with walker or wheelchair as instructed.
• Assist with prescribed treatments and self-administered medications when allowed.
• Encourage independence per care plan; monitor and report changes to nurse.
