Qualifications

• Proven ability to communicate effectively with clients and families.
• Ability to follow verbal and written care plans precisely.
• Experience using Care management software for documenting visits.

Responsibilities

• Assist clients with ADLs as per care plans (bathing, dressing, grooming, etc.).
• Provide medication assistance, housekeeping, meal prep, shopping, and transportation support.
• Report daily activities and changes to supervisor via system and other channels.
• Maintain confidentiality and HIPAA compliance.
