Job summary

Amivie, LLC is seeking a Personal Care Attendant to assist clients with Activities of Daily Living (ADLs) and Instrumental ADLs (IADLs) in home-based settings. You will follow the care plan, report changes to the RN Field Supervisor, and adhere to all company policies.

The role requires reliability, professionalism, and compassionate client care. Responsibilities include maintaining scheduled services, accurate time reporting (EVV/timesheets), and ensuring safety with PPE and nametag use.

Qualifications

• Eighteen (18) years of age or older.
• Criminal history check compliant with 6.0 Clinical Coverage Policy 3L.
• No finding on the North Carolina Health Care Registry per G.S.131E256.
• Experience in Home and Community Based Services, preferred.
• Current Tb Skin Test.
• Desire to help those at home.
• Professional demeanor and appearance.
• Have reliable transportation and proof of current automobile insurance.
• Can demonstrate competence providing assigned tasks.

Responsibilities

• Comply with all policies and procedures in the orientation manual and materials during employment.
• Provide services as assigned by the RN Field Supervisor per the individualized care plan.
• Report deviations in a client’s condition to the appropriate agency personnel.
• Deliver services according to the assigned schedule.
• Notify the agency of deviations, and unexpected events (e.g., hospitalization, facility placement).
• Demonstrate competence with ADLs and IADLs per the plan.
• Be present for Supervisory Visits by the RN Field Supervisor.
• Accurately report time and attendance via timesheets or EVV as required.
• Report concerns of Abuse, Neglect, or Exploitation to the office.
• Wear nametag and PPE as required; dress professionally.
