Job summary

Amivie, LLC in Fuquay-Varina, NC is seeking a Personal Care Attendant to provide ADLs and IADLs per the individual care plan, primarily in the home setting.

You will follow the RN Field Supervisor's directions, document time and attendance, and report any deviations or concerns promptly.

The role requires 18+ years, a clean background, reliable transportation, and a professional demeanor with compassionate, person-centered care.

Qualifications

• Eighteen (18) years of age or older.
• Criminal history check compliant with 6.0 Clinical Coverage Policy 3L.
• No finding on the North Carolina Health Care Registry per G.S.131E-256.
• Experience in Home and Community Based Services preferred.
• Current Tb Skin Test.
• Punctuality and reliability.
• Have reliable transportation and proof of current automobile insurance.
• Ability to demonstrate competence providing assigned tasks.

Responsibilities

• Comply with all policies and procedures in the Quality Home Staffing / Community Based Home Care Employee Manual.
• Provide services as assigned by the RN Field Supervisor in accordance with the individual’s approved service plan.
• Report deviations in an individual’s functional or medical condition to the appropriate agency personnel.
• Provide services during the assigned schedule for each individual.
• Report deviations from the assignment, including unassigned tasks or unavailability.
• Report incidents, accidents or other events not aligned with the plan of care.
• Demonstrate competence with assigned ADLs and IADLs (e.g., bathing, dressing, mobility, toileting, eating).
• Demonstrate competence with IADLs (e.g., cooking, cleaning, shopping, transportation, escort).
• Provide companion or sitter services as assigned.
• Notify RN Field Supervisor for any additional services or alterations.
• Attend supervisory visits by RN Field Supervisor and log time/attendance via timesheets or EVV.
• Report concerns of Abuse, Neglect or Exploitation immediately.
