Job summary

YAI is seeking dedicated direct support professionals to assist individuals with intellectual and developmental disabilities. You will provide emotional support, promote social connections, and help people follow personalized service plans.

The role requires safeguarding, data collection, and coordination of medical appointments. Responsibilities include implementing Behavior Support Plans, maintaining safety, and working across multiple settings with a collaborative team to ensure high-quality

Qualifications

• Must be at least 18 years old.
• HS diploma or equivalent; and
• Valid Driver’s License with no more than five points.
• Able to meet physical demands including lifting and assisting with movement.

Responsibilities

• Assist individuals in maximizing strengths and abilities with person-centered support.
• Provide health, wellness, and safety supervision and manage crisis situations.
• Maintain accurate documentation and communicate with team and families.
• Operate agency vehicle and coordinate appointments as needed.
• Implement Behavior Support Plans and ISP guidelines as per plans.
