Job summary

Redwood Coast Medical Services is seeking a compassionate Registered Nurse to support our Visiting Nurse and Palliative Care Programs along California’s coastal region. You will provide skilled nursing care, patient education, and care coordination for homebound and transitioning patients.

Responsibilities include assessments, medication management, and collaboration with providers and families to implement individualized plans.

Qualifications

• Current California RN license.
• BLS certification.
• Valid driver’s license and reliable vehicle.
• Strong assessment and documentation skills.

Responsibilities

• Provide skilled nursing care, assessments, medication management, dressing changes, and patient education.
• Support homebound patients, hospital-to-home transitions, and life-threatening or end-of-life illnesses.
• Collaborate with primary care providers and families to develop individualized care plans.
• Teach patients and caregivers on managing acute medical needs and navigating services.
• Coordinate care and facilitate access to in-home assistance and community resources.
• Promote patient dignity, comfort, and informed participation in healthcare decisions.
• Monitor patient status and communicate changes to care teams.
• Maintain accurate documentation per regulatory standards.
• Travel across RCMS service area to provide care.
