Job summary

National Church Residences is seeking a Medical Social Worker to provide compassionate social work services in a Southern Ohio home health setting. You will support seniors and families, coordinate referrals, and participate in care planning with a multi-disciplinary team.

Qualified candidates hold a CSWE-accredited degree (BSW/MSW) and an LSW in good standing, with 1–2+ years of healthcare social work experience.

Qualifications

• Bachelor’s or Master’s in Social Work from a CSWE-accredited program.
• LSW in good standing is required/preferred.
• Minimum two years in varied healthcare settings with counseling experience; at least one year in healthcare social work.

Responsibilities

• Provide medical social work services to seniors in home health/hospice.
• Participate in care planning and discharge planning with the care team.
• Counsel patients and families, facilitate coping and adjustment.
• Coordinate resources and referrals to support services.
• Participate in on-call rotation as required.
