Job summary

National Church Residences Home Health in Chillicothe, OH is seeking a Medical Social Worker for a full-time, Monday through Friday schedule with an on-call rotation. You will support seniors and families, coordinating care and resources to improve quality of life.

The ideal candidate holds a Bachelor’s or Master’s in Social Work (CSWE-accredited) and an active LSW. At least one year of health care social work is preferred, with 2+ years in multi-disciplinary settings.

Education

• Bachelor’s or Master's Degree in social work from a school of social work accredited by the Council on Social Work Education.
• LSW in good standing.

Experience

• Preferred minimum of two (2) years in varied multi-disciplinary settings in the health care field with experience working with clients and family counseling; however, requires one year of social work experience in a health care setting.
