Job summary

Good Shepherd Hospice is seeking a compassionate PRN Social Worker to serve patients and families in Enid and surrounding communities. The role provides social work services aligned with the interdisciplinary Plan of Care and requires collaboration with the care team to support optimal psychosocial outcomes.

Qualified candidates hold an MSW (CSWE-accredited) or a related BA with MSW supervision, and licensure in the delivering state.

Qualifications

• MSW accredited by CSWE is required.
• BA in social work, or related field, with MSW supervision.
• Licensed in the state where care is delivered (if required).
• At least one year of social work experience in health care.

Responsibilities

• Assist care team in addressing social and emotional factors affecting health.
• Assess social and emotional needs within 5 days of admission.
• Help patients and families understand and follow medical recommendations.
• Coordinate resources from family and community agencies to support patients.
• Document services in the patient’s clinical records accurately and timely.
• Participate in Interdisciplinary team meetings and Plan of Care reviews.
