Symbii Home Health and Hospice-Layton is seeking a Social Worker to implement standards of care and the patient’s plan of care for medical social work services in home health and hospice settings.

The role includes assessing psychosocial status, counseling patients and families, maintaining records, coordinating with the interdisciplinary team, and connecting patients with community resources to support quality end-of-life care.

Qualifications

• Assesses psychosocial status of patients and families/caregivers and communicates findings to the care team.
• Maintains clinical records on all patients referred to social work.
• Provides information and referral services to patients and families/caregivers.
• Acts as liaison between patients, families, and community agencies.

Responsabilités

• Assess psychosocial status and plan interventions.
• Counsel patients and families to manage stress and coping difficulties.
• Maintain and develop patient care plans within the interdisciplinary team.
• Maintain and assist the volunteer coordinator in supporting hospice services.
• Participate in quality improvement activities.
