Three Oaks Hospice, Inc. is seeking a licensed social worker to provide coordinated in-home psychosocial assessments, counseling, and referrals for patients across all ages.

You will complete timely comprehensive assessments, assist with advance directives, and support families through end-of-life planning. The role requires LMSW licensure or eligibility, a solid knowledge of community resources, and strong interpersonal skills.

Qualifikationen

• Bachelors or master's degree in social work from a state-accredited program.
• Current LMSW license or eligibility to obtain licensure.
• Working knowledge of community resources; strong interpersonal skills.
• Hepatitis profile and valid driver’s license as required by policy.

Aufgaben

• Provide coordinated in-home psychosocial assessments, analysis, counseling, and referrals.
• Complete the Comprehensive Assessment within five days of hospice election to identify needs.
• Assist with advance directives, funeral planning, discharge planning, and family counseling as needed.
• Use effective verbal and written interpersonal communication skills.
• Meet mandatory continuing education requirements for hospice and licensing board.
• Demonstrate commitment and ongoing professional development.
• Provide counseling to patients and families about end-of-life, death, and grief.
• Promote hospice philosophy and administrative policies to ensure quality of care.
• Attend IDG/IDT meetings; coordinate patient care across teams.
• Accept clinical assignments aligned with education and competence to meet patient needs.
• Create an environment respecting patient privacy and property.
• Participate in the hospice QAPI program and educational in-service programs.
