Three Oaks Hospice seeks a Full Time Licensed Masters' Level Social Worker to provide coordinated care in patients’ homes, performing psychosocial assessments and referrals to meet patient and family needs.

The role requires a Master’s in social work, a current LMSW license, reliable transportation, and knowledge of community resources, with commitment to compassionate end-of-life support and team collaboration.

Qualifications

• Master’s degree in social work from a State accredited program.
• Current and valid LMSW license.
• Working knowledge of community resources. Good interpersonal skills. Hepatitis profile.
• Current State Certification.
• Reliable transportation. Valid Driver’s License and current auto liability insurance.

Responsibilities

• Provide coordinated care in patients' homes and perform psychosocial assessments and referrals.
• Assist with advance directives, funeral planning, discharge planning, and family counseling.
• Use effective interpersonal communication, written and oral.
• AttendIDG/IDT meetings and participate in patient care planning.
• Promote hospice philosophy and administrative policies; ensure quality of care.
• Participate in Hospice’s QAPI program and maintain infection control standards.
