Job summary

Harmony Hospice seeks a dedicated Hospice Social Worker to deliver psychosocial support, counseling, and resources to patients and families navigating end-of-life care in Rhode Island. This full-time role requires MSW, RI licensure, and healthcare experience, with CPR certification and a valid driver's license driving community-based services.

Collaborate with an interdisciplinary team to address patient goals, provide coping strategies, and ensure compliant documentation.

Qualifications

• MSW from CSWE-accredited program.
• Current Rhode Island social work license required.
• Bachelor's degree in Social Work required.
• Previous healthcare or hospice experience preferred.
• Strong communication and organizational skills.

Responsibilities

• Provide psychosocial support and counseling to hospice patients and families.
• Assess psychosocial needs and develop support plans.
• Collaborate with interdisciplinary team on care goals.
• Educate on resources and coping strategies.
• Maintain accurate documentation of services.
• Support patients and families through illness, loss, and grief.
