Job summary

VITAS Healthcare in California is seeking a hospice social worker to provide emotional and logistical support to patients and their loved ones during a vulnerable time.

You will help patients ensure their affairs are in order, honor their wishes for finances and funeral arrangements, and guide families through grieving, while collaborating with an interdisciplinary team across home and office settings.

Qualifications

• MSW from CSWE-accredited program or grandfathered in under Medicare Hospice conditions.
• At least one year of social work experience in a health care setting.
• Experience with terminally ill patients and families is preferred.
• Ability to provide counseling, crisis intervention and bereavement support.

Responsibilities

• Provide emotional and logistical support to patients and loved ones.
• Help families organize finances, funeral arrangements, and patient affairs.
• Assist patients in navigating end-of-life planning and grieving processes.
• Collaborate with interdisciplinary team to deliver 360-degree support.
