Job summary

Geisinger Hospice-Lewistown, part of LHC Group and within the Optum family, seeks a Social Worker to provide medical social services under physician direction and an Interdisciplinary Group. You will help patients and families understand social and emotional factors related to health and support coping strategies.

Responsibilities include forming a Plan of Care with the health care team and guiding colleagues on local resources to assist patients as needed.

Qualifications

• Master's Degree from a CSWE-accredited school of social work.
• Current driver's license and access to reliable transportation.

Responsibilities
