Job summary

Gentiva is seeking an After Hours Hospice RN to respond to patient needs during evenings, nights, weekends, and holidays, delivering compassionate care and peace of mind in patients' homes or facilities. This full-time role includes a Baylor weekend schedule with benefits and PTO.

You will report to the Administrator or Patient Care Manager, provide direct patient care after hours, coordinate with the hospice team, document visits, and participate in quality improvement programs while

Qualifications

• Current RN license in the state of employment.
• 1+ years of nursing experience.
• Hospice heart: compassionate, calm, dependable.
• Valid driver's license and reliable transportation.
• Current CPR certification.

Responsibilities

• Report to Administrator/Executive Director/Patient Care Manager Senior.
• Provide direct patient care after normal business hours.
• Serve as liaison between patients, families, physicians and the hospice team.
• Respond promptly to urgent calls, visits, and symptom needs.
• Evaluate patient/family needs and guide clinical decisions.
• Collaborate with LPNs/LVNs, CNAs and other staff.
• Document clinical visits and communications accurately.
• Participate in quality assessment and performance improvement.
