Job summary

Three Oaks Hospice is seeking a PRN On-Call RN to join our hospice care team serving clients in the West Valley area. The successful candidate administers skilled nursing care, coordinates with the inter-disciplinary team, patients, families, and referring agencies, and ensures seamless care delivery in patients' homes.

The role requires an RN license, CPR certification, and 2 years of clinical RN experience (hospice/home health preferred).

Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state RN license and current state driver’s license.
• Two years’ RN experience in a clinical setting; hospice or home health preferred.
• Current CPR certification.
• Proficient computer skills; MS Office experience preferred.
• Strong interpersonal skills to support patient and family.

Responsibilities

• Administer skilled nursing care for clients in their place of residence.
• Coordinate care with the interdisciplinary team, patient/family and referring agency.
• Assume responsibility for coordination of care.
