Job summary

Three Oaks Hospice is seeking a passionate PRN On-Call Registered Nurse (RN) to join our team of hospice care professionals. You will provide skilled nursing care to patients in their homes, coordinate with an interdisciplinary team, and support families with compassionate, patient-centered services.

The role emphasizes on-call flexibility, strong clinical skills, and the ability to manage complex emotional situations while maintaining high-quality care standards.

Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state license as an RN and a valid driver’s license.
• Two years’ RN experience in a clinical care setting; hospice or home health preferred.
• Current CPR certification.
• Proficient computer skills; Microsoft Office experience preferred.
• Strong interpersonal skills and ability to support patients and families.
• Demonstrate nursing practice with ability to cope with family emotional stress and varying lifestyles.

Responsibilities

• Administer skilled nursing care in clients’ homes.
• Coordinate care with the interdisciplinary team, patient/family, and referring agency.
• Coordinate plan of care and modify as needed.
• Participate in on-call coverage and after-hours support when required.
• Maintain accurate documentation and communication with the care team.

We are looking for a passionate PRN On-Call Registered Nurse RN to join our leading team of hospice care professionals and make a difference, by listening, caring, and serving.

POSITION SUMMARY: Primary functions are to administer skilled nursing care for clients of all ages in their place of residence, coordinate care with the interdisciplinary team, patient/family and referring agency; and assumes the responsibility for coordination of care.

Qualifications
