Job summary

Responsibilities include initial admissions assessments, care plan coordination, progress updates to the team, and maintaining accurate documentation while adhering to center policies.

Qualifications

• 1+ years of nursing experience.
• Current CPR certification.
• Valid driver's license, reliable transportation, and liability insurance.

Responsibilities

• Performs initial nursing assessment on new admissions.
• Coordinates and revises the plan of care with the clinical team.
• Communicates patient progress to the clinical manager and care team.
• Maintains patient care records with accuracy and timeliness.
• Educates families on patient condition and needs.
