UnitedHealth Group is seeking a Per diem Hospice Registered Nurse to provide compassionate, patient-centered home health care. You will assess needs, coordinate individualized care plans, manage symptoms, educate families, and collaborate with the clinical team in a per diem capacity.

The role requires an active RN license for the state(s) assigned, CPR certification, and a valid driver’s license with reliable transportation.

Qualificações

• Current RN license in the state(s) you are assigned.
• CPR certification current.
• Valid driver’s license, reliable transportation, and liability insurance.
• Preferred: 1+ years of nursing experience.

Responsabilidades

• Performs initial nursing assessment on new admissions.
• Completes, coordinates, and revises the total plan of care with the care team.
• Regularly communicates patient progress to the clinical manager, physician, and care team.
• Educates families on patient condition, needs and care.
• Makes referrals to other hospice care disciplines as needed, or documents rationale for not doing so.
• Performs on-call responsibilities and on-call services to patients/families as assigned.
