Job summary

Liberty Hospice is seeking an experienced Hospice Registered Nurse (RN) for per diem shifts in Sampson/Wayne counties, North Carolina. You will apply nursing theories, document care per reimbursement guidelines, and coordinate physical care with family, patient, physician, and the Hospice team.

The role requires RN licensure, 2+ years of nursing experience with hospice preferred, CPR, and willingness to travel with on-call weekend/evening/holiday assignments.

Qualifications

• Current RN licensure in state of practice.
• Two years clinical nursing experience; hospice preferred.
• Knowledge of home health/hospice care framework and reimbursement guidelines.
• 12 hours of continuing education annually.
• CPR certification required.
• Willing to travel and take weekend/evening/holiday on-call.

Responsibilities

• Apply nursing theories and concepts in patient care.
• Document care meeting reimbursement guidelines.
• Coordinate physical care with family, patient, physician, and Hospice team.
• Communicate with patients and families at all levels.
• Communicate with agency personnel and attend IDT meetings.
• Participate in quality reviews as scheduled.
